Student Exemptions

A student may be exempt from this requirement under one of the following circumstances:

  • The vaccine is contraindicated for the individual based on guidelines established by manufacturer or Center for Disease Control

Students must submit documentation from health care provider for medical contraindications.

Students must submit written documentation from religious leader for religious contraindications.
